Bluewater Health's ACT program provides treatment, rehabilitation and support services for individuals with severe and persistent mental illness in Sarnia who require intensive service to live independantly in the community. Individualized services that address mental and physical health, addiction, education, vocation, social development and functioning activities of daily living and family, and other relationships, are provided to the client in their home and the community. Using an integrated approach by a multi-disciplinary team of mental health professionals, ACT works with clients to reach their individual goals. Services are available to ACT clients seven days a week.
