The Rapids Family Health Team is a support system of many allied health professionals designed to work with your primary care provider to provide you with the highest possible quality of care. Our Registered Nurse Educators and Registered Dietitians provide patients with programs and counseling for managing Diabetes, Nutrition and Dietary issues, COPD, Congestive Heart Failure, how to stop smoking and more. Mental Health Workers offer individual counseling and groups to help patients manage depression, anxiety, stress and more. Other services include our Access to Care Centre located at 481 London Rd., Sarnia, patients who are unable to see their Primary Care Provider are treated by Nurse Practitioners for episodic care and a COVID, Cold and Flu Care Centre located at 1150 Pontiac Dr. Sarnia
